Every process server must keep the following information for each service they make for 2 years:
- The title of the action (case caption)
- Names of the persons served (if known)
- Date and approximate time of service
- If served by Nail and Mail, the dates and times of the attempted services
- Address where service was effected
- Nature of the papers served
- Court in which the action has been commenced
- The Index # of the action (if known)
Additionally, if a person was served, the process server should keep record of a description of the person including:
- Sex
- Skin Color
- Hair Color
- Approximate Age
- Approximate Height
- Approximate Weight
- Other Identifying Features
If you serve a person by Drop Service or Intercoms and Video Doorbells, in place of a description of the person, simply note “Held conversation with person served” followed by a narrative of the service event.
